Companion Care

Companion Care focuses primarily on the client and his or her social needs. Companions offer a wide range of services to clients, including, but not limited to:

  • Providing socialization, friendship and support
  • Assistance with physician appointments
  • Walks and encouraging stimulating activities
  • Supervising ambulation and fall prevention
  • Accompany to doctor’s appointments
  • Shopping and other household errands
  • Providing hygiene assistance
  • Reminding clients to take their medication
  • Laundry and bed linens changing
  • Perform light household tasks (meal plan/preparation, laundry, straighten living areas, etc.)

Personal Care

Sometimes a family member requires a licensed health professional who can attend to more personal needs. Elderly persons often require personal assistance but are embarrassed to permit family members to assist. We can help by arranging a Personal caregiver (Certified Nursing Assistant) to assist with all the tasks of Companion Care PLUS:

  • Assist with “hands on” care, including bathing, dressing, skin, mouth and hair care
  • Measure temperature, pulse, respiration, blood pressure, intake and output of fluids (if necessary)
  • Walks and encouraging stimulating activities
  • Assist with prescribed range of motion (ROM) exercises
  • Assist with the use of ambulation devices (wheelchair, walker, etc.)
  • Assist with the change of a colostomy bag, reinforcement of dressing, etc.
How Can I Obtain These Services?

How Long Will The Process Take?

It usually takes 1-2 month(s) from the date of the initial call.  We refers the case to the local Area Agency on Aging (AAA).  The AAA assessor evaluates the client’s health condition.  If the client qualifies, we will receive a service order from the AAA to send a caregiver to the client’s home for a specified number of hours per week.

How Much Does It Cost?

Services are available at little or no cost to low income individuals who qualifies for Medicare Waiver, SOURCE or CCSP Program. 

SOURCE

Service Options Using Resources in a Community Environment (SOURCE) is a Medicaid Waiver Program offering an array of services to help persons stay in the community. It is similar to the Community Care Services Program, but only serves individuals who have SSI or Public Law Medicaid. It is a very cost effective program in comparison to nursing home care.

CCSP

The Community Care Services Program is a Medicaid Waiver Program offering an array of services to help persons stay in the community. It serves individuals with limited incomes and resources and is a very cost effective program in comparison to nursing home care. It is similar to the SOURCE Waiver program, but does not require an individual to use a specific primary care physician.
